Patient Interview

To interview the patient, click on the questions below in any order you choose.

Would it be okay, Ms. Harris, if we spend a few minutes talking about your brain health, including how your memory and thinking have been lately?
Do you have any concerns about your memory or thinking?
Can you walk me through a typical day for you, from when you wake up to when you go to bed?
I’d like to ask you about your mood and social life. Have you noticed any changes in how you’ve been feeling, or in your interest in staying connected with others?
When patients come in alone, I sometimes ask whether there’s someone, like a family member or close friend, who could share their perspective on how you’ve been doing? Is there anyone you’d feel comfortable involving?
